    Treatment & prophylaxis of venous thromboembolism; prevent clotting during extracorporeal circulation
    Dosage ：
    Treatment of acute deep-vein thrombosis 200 iu/kg SC once daily, up to a max of 18,000 iu. Alternatively, a dose of 100 iu/kg SC bid may be used. Prevention of clotting in the extracorporeal circulation during hemodialysis or hemofiltration: >4 hr 30-40 iu/kg by IV bolus inj, followed by 10-15 iu/kg/hr IV infusion; max of 4 hr As above or 5000 iu as a single IV bolus inj.
